Performs duties by working with physicians to identify and carry out treatment plans. Responsibilities Works with the Medical Director and other physicians to identify and carry out the treatment plan. Acts as a Patient Advocate helping to navigate patients through the diagnosis, treatment and rehabilitation process. Serves as a resource to assist patients and families, other professionals and the community. Provides education for the community and referring physicians. Facilitate clinical protocols established by Medical Director(s).
