<h2>Overview</h2> <p>Presbyterian Homes & Services – Optage Hospice is seeking an experienced RN or LICSW looking to use their knowledge and skills in a leadership position as a <strong>Hospice Clinical Supervisor</strong>.</p> <p> </p> <p>This position is ideal for those who feel called to hospice and wish to serve older adults within a caring Christian environment. As a part of Presbyterian Homes & Services, we are a mission driven, faith-based organization with a strong and supportive team culture, pledging to do the right thing, because it is the right thing to do.  </p> <p>Whether you have years of supervisory experience or are new to a leadership role, if you have a passion for hospice and enjoy working with older adults, we would love to talk with you!</p> <p> </p> <h2>Responsibilities</h2> <p>The Hospice Clinical Supervisor will serve as a leader providing supervision for a team of professional registered nurses.  This position has responsibility as a member of the hospice management team to participate in the planning and implementation of strategies and programming that ensure high quality client care while achieving short and long-term operational goals and objectives for the Hospice program.</p> <p> </p> <p>In conjunction with the essential functions of the clinical supervisor, this addendum displays the areas of focus and responsibilities for the oversight of the HOSPICE ADMISSIONS & VISIT NURSE TEAMS.</p> <p> </p> <p>1. Referral, Intake, and Admissions Oversight</p> <p>• Provide primary oversight of referral intake and admissions coordination to ensure timely, accurate, and complete referral workups.• Balance intake workloads, ensuring appropriate staffing coverage and support.• Ensure documentation is completed accurately and within required timeframes.• Facilitate timely, clear communication across all stakeholders throughout the admissions process.</p> <p>2. Visit Nurse & Admissions Nurse Team Management</p> <p>• Ensure nurses are cross‑trained to support both visit and admissions responsibilities.</p> <p>3. Transfers, Re‑Admissions, and Face‑to‑Face Oversight</p> <p>• Oversee hospice transfers and re‑admissions, ensuring all required records are obtained, reviewed, and accurately entered into HCHB.• Coordinate and secure nurse practitioner or physician support for required face‑to‑face visits related to re‑admissions.• Communicate re‑admission plans clearly to admissions staff and service coordinators.• Identify and deploy appropriate clinical resources for in‑person assessments as needed.• Ensure coroner pre‑registration processes are completed in accordance with policy when applicable.</p> <h2>Qualifications</h2> <ul> <li>Current professional licensure as an RN, MSW, or LICSW in state(s) of practice and oversight.Bachelor’s degree or higher, preferred.</li> <li>Three to five (3 - 5) years of leadership experience in Home Care/Hospice, preferred.</li> <li>Demonstrated clinical competencies in home care/hospice care management and operations, quality and business management, team building and employee development, client and family support systems, governmental regulations and community outreach.</li> <li>Demonstrated knowledge and experience in hospice and palliative care or home care or clinical practices.</li> <li>Must have valid driver’s license and proof of auto insurance.</li> <li>Demonstrated computer proficiency, including Microsoft Office Suite and electronic medical records (EMR).</li> <li>Demonstrated compatibility with PHS's mission and operating philosophies.</li> <li>Demonstrated ability to read, write, speak, and understand the English language to communicate with all customers.</li> </ul> <h2>Pay Range</h2>The estimated starting pay range for this role is $68,000-$101,982 yearly. This good faith estimate is based upon PHS’s typical starting pay range for this role.
